Conclusion

Britain is justly proud of its NHS. It provides a high-quality, though not flawless service, and is cost-effective, though, as mentioned above, it is essentially an illness service rather than a health service. It is not without problems, but neither is it without successes. Perhaps the most serious problem it faces is that of almost continuous change with scarcely any opportunity to assess the effects of the change, and the need to adapt to the perceived health needs of an increasingly multicultural, multi-ethnic society. To date, there is little evidence of this adaptability.

Most of the issues discussed in this chapter are far from new, though many have only come to public awareness in relatively recent times. There is no doubt, for instance, that rationing, by a variety of covert or overt means, has been a facet of the NHS since its inception. Similarly, the question of whether doctors should be employees of the NHS has been in contention since the 1930s. However, whilst these issues may excite academic interest and research, it is questionable whether they are of such interest to the service users. Accessibility and appropriateness of service provision are of greater concern to the individual, and the number of people on a waiting list will be of little significance for the older person awaiting surgery for hip replacement; they will be interested in the length of the wait.
